Job Description

Administers Dementia Programs to meet the specific needs of each resident and all residents within the Memory Care Neighborhood. Ensures residents lives are maintained to the extent possible by providing and coordinating activities and contact with the community. Ensures residents are treated with dignity and respect, recognizing the individual needs and encouraging independence.

1. guidelines that will meet the individual needs and interests of the residents to include cognitive/mental stimulation, physical activities, productive work activities, social, sensory, reflective activities, outdoor and nature related activities. Plans a seven-day per week calendar that promotes resident participation in a structured environment including one-to-one programming for residents who cannot participate in a group setting. Ensures programs meet all states, federal and other regulations.

2. Assists with completion of the resident s social history upon admission or as directed by nursing or administration.

3. Routinely provides program directions, materials and resources for staff to ensure their success when facilitating programs.

4. Assists with planning, coordinating and appropriately decorating the communities for special events, seasons, holidays and educational programs.

5. Provides guidance and acts as a resource to staff on the importance of programming/life enrichment and other areas regarding quality of life for residents.

6. Assists with coordination and participates in special events at the community including family socials and open houses. Develops and maintains contact with community agencies and organizations.

7. Communicates positively with residents, families, and community professionals regarding various programs offered within each community and their benefit.

8. Organizes and assists with facilitation of resident and family committees to work on issues of planning of programs and volunteer needs.

9. Assists with collaboration of care plans, UAI s and ISP as required by state and federal regulations or at change of resident condition.

10. Reports changes in residents overall health to the nurse, including changes in behaviors and takes actions to address concerns in a timely manner.

11. Assist with the coordination of transportation of resident to and from scheduled events outside of the facility, which may include driving the community motor vehicle.

12. Participates in monthly quality assurance improvement meetings where applicable.

13. Assists in tours, telephone support, and customer contact.

14. Manages volunteer program, including training and supervision.

15. Flexible schedule to provide support to community in absence of other activity professionals.

16. Performs personal care, housekeeping and dietary responsibilities as needed to ensure service quality is maintained.

17. Assist with medication administration where applicable with appropriate licensure.

18. Completes other duties as assigned by Administrator, Director of Nursing, Corporate Compliance Officer and/or CEO or designees.

Education and Experience

You must have previous experience working with those living with dementia Alzheimer s and other cognitive impairment.

Certifications, Licenses, and other Special Requirements

A preferred Certified Dementia Practitioner. Required UAI and ISP certification.

Management and Decision Making

Applies existing guidelines and procedures to make varied decisions within a department. Uses sound judgment and experience to solve moderately complex programs based on precedent, example, reasonableness or a combination of these. Understands chain of command that should be used to address concerns or problems.
